Meet the Intake Form: an application form that doesn't look like a Kajabi form

It started with someone asking for a "fancy" intake form

I get a lot of questions through the Lab's suggestion box. Most of them are "can the Lab do X?" and I quietly add them to my list.

But this one made me laugh out loud… because it was TWO people, in the same week, asking for basically the same thing from two different directions.

One of them runs a certification program and wanted an application form that felt like a proper application. A real page. Their branding. Not a box of fields squished into a section.

The other one was setting up an intake form for a cohort and asked, very politely, "how would you recommend using the Library to make my form look… branded?" 🫣

And I sat there thinking… you know what… I have ALL the pieces for this already. The Pop Quiz builder already makes beautiful question pages. It already makes the page where people leave their name and email. It already writes emails and lands everything in your Kajabi account as normal pages.

It just always assumed you wanted a result at the end.

So I took the result out.
And BAM… the Intake Form was born. 

Let's be honest about the normal Kajabi form for a sec

I love Kajabi. You know I love Kajabi. But a native form is a box. It sits inside a section, it looks like every other Kajabi form on the internet, and the moment you ask more than three questions it turns into a very long, very grey scroll.

Which is fine for "pop your email in for the freebie."

It is NOT fine for "tell me about your business, your goals, what you've tried, and why you want to work with me." That's a conversation. It deserves a page that feels like one.

What the Intake Form actually is

It's the fourth thing you can build in Pop Quiz, next to the quiz, the personality quiz, the scored quiz and the assessment. Same builder, same steps you already know… just no scoring, no results, no "you got Type B."

You write your questions, choose how the page looks, say what happens when someone hits send, and the Lab builds the whole thing into your Kajabi site:

• the form page(s) with your questions on them
• the send page where they leave their name and email
• the thank-you page with a "here's what happens next" line and a button
• a confirmation email drafted for you, in an editable Kajabi email
• a tag that goes on every person who submits
• the Kajabi form itself with a field for every question, sitting in your Kajabi forms list like any other

One button. Every piece. Already wired to each other. 🙌

(That last line is the whole reason I built the Lab... because remember.... Kajabi gives you the pieces. The Lab puts them together for you.)

Where you'd actually use this

This is the bit that got me excited, because once the result page was gone… the list of things it could be just kept growing.

An application form for your program.
"Tell me about you, your business, where you're stuck." Someone applies, you get the answers, you decide who's in. Perfect for cohorts, masterminds, certifications, anything with a door you keep half-closed on purpose.

A discovery-call intake.
Ask your questions first, THEN send them to your calendar. The thank-you page can point straight at your Calendly (or whatever you use), so they answer, hit send, and book… in that order. No more calls where you spend the first ten minutes asking what they already told you in an email.

Client onboarding.
New client says yes. Send them one link. They fill in the brand colours, the logins, the "what do you want this to look like," and it's all sitting in their Kajabi contact record before your kickoff call.

A retreat, workshop or event registration.
Dietary stuff, arrival times, what they're hoping to get out of it, room preferences. All the little questions that usually come back in eleven separate emails.

A "what should I make next?" survey.
Ask your audience. Tag everyone who answers. Follow up with the people who said the thing.

A guest or speaker application.
Podcast guests, summit speakers, collaborators. Topic, bio, links, availability. Done.

Basically… any time you've ever typed "just fill in this quick Google Form" and then felt a little bit embarrassed about it. 

Pick your look: one page, or one question at a time

This was a new one, even for the quizzes, and I think it's my favourite part.

All on one page.
Every question, numbered, on a single clean page with a big send button at the bottom. This is the classic application-form feel. If someone skips a question the page highlights it, gives them a little nudge and scrolls them to it. No mystery "something went wrong."

One question at a time.
One question fills the screen, they answer, the next one slides in. It's the look you'd normally pay a monthly fee to one of those fancy form tools for… except it's a Kajabi page, on your site, and you already own it.

Same questions, same everything. Flip a switch, update in place, done. Try both and see which one your people finish more of.

Your logo, your words, your language

Your logo pulls straight from your Kajabi media library and sits at the top of every page.

Every word on the page is yours. The "Almost there!" on the send page, the button labels, the "Next step:" on the thank-you page… all of it is editable, with a little live preview beside each one so you can see where it lands.

Which also means… if your audience isn't English-speaking, neither is your form. Nothing is baked in. (Hi to my Danish and Swedish Lab members 💕)

After it's live (ooohh yeah...)

The pages the Lab builds are NORMAL Kajabi pages.

Open one in the Kajabi editor and it's all there: your header, your footer, and the form sitting in its own section. Want a bit of intro text above the questions? Add a text block. Want your face on the send page? Add it. Want the thank-you page to also show your three best testimonials? Go for it.

The only rule is the same one as The Runway: pick one home for your edits. Change the questions in the Lab and push an update, and the Lab rewrites its own sections (it warns you first, and anything you've added yourself in Kajabi is left alone). Fine-tuning in Kajabi? Stay in Kajabi.

Two little things that make it work properly

The tag.
Everyone who submits gets a tag, which is how you follow up. The Lab writes the tag into the form for you, and then shows you the ONE Kajabi automation to set up so it actually fires… form submitted, add the tag, no conditions, done. (Yes, this is still a manual step. Kajabi hasn't opened automations up to tools like the Lab yet. I'm waiting. Patiently. Ish. 😅)

Single opt-in.
Your intake form MUST be set to single opt-in in Kajabi. Double opt-in means the tag and the automation wait for someone to click a confirmation link, and for an application form that's just… weird. The Lab reminds you of this when you build it, but I'm reminding you here too because I know you. 😉
